The Bloomhouse Women’s Counseling Collective (“The Bloomhouse,” “we,” “us,” or “our”) is committed to protecting the privacy and confidentiality of your protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA).

4. Appointment Reminders and Communication

We may contact you by phone, email, or text to remind you of appointments or communicate about services.

You may request alternative communication methods.

5. As Required by Law

You may request a paper copy of this Notice at any time.

Minors and Parents

In accordance with Colorado law, minors may have certain rights to confidential mental health services. Access to records by parents or guardians may be limited in some circumstances.

We will follow applicable state laws regarding minor consent and confidentiality.

Breach Notification

In the event of a breach involving unsecured PHI, we will notify affected individuals in accordance with federal and state requirements.
